I want to make a mystery shopping research for a relative that has small chain of shops about mobile phones %26amp; accessories. We listed some questions both quantitative and qualitative But we still need some sample forms, questions or sections that should be included in the form. Could you please give some examples, or make some suggestions for survey design?

I suggest you join the Yahoo! Group mystery-shopper . It is a group of shoppers, schedulers, and mystery shopper companies. Once you have joined and your membership has been approved, you could ask this question of these experts! Mystery shoppers don't make up the questions asked but they can surely provide you some examples of questions asked and provide you some examples of some of the dumbest questions asked.
Reply:i think your survey should be accurate and concise. Accurate means you will get results from that paper; concise means simple design making your customer a ease to fill the form, better all selective questions
